Internal Affairs Initial Inquiry

  1. We ask that you provide the time and location of the event, the name of the employee(s) involved, and, if possible, the employee's badge number or description of the employee, along with a detailed description of the event. Please include as much detail as possible.

    In most cases, you will be required to formalize your complaint in a sworn affidavit. This may be done by mailing a signed letter detailing your complaint or making an appointment with the Internal Affairs Division to give your statement in writing. This form is only the first step in the complaint process. Bear in mind that you may be required to provide additional written information as part of the investigation process.
